Albany Care Center Takes New Approach to Improve Diabetes Care

On October 16, Sutter East Bay Medical Foundation welcomed patients with diabetes to an interactive Diabetes Health Fair at its Albany Care Center. The Diabetes Health Fair brought the Patient Centered Medical Home concept to life through a “one-stop shop” of services and education designed to help patients self-manage their medical conditions.

“Patients with diabetes have to stay involved in their care and it can be hard to get patients to manage their condition,” said Nina Birnbaum, M.D., care center site lead. “We wanted to come up with a way to support patients and help them take care of themselves.” Read More
